CarePort Health will take over management of its sister company Allscripts Care Management’s portfolio of case management, utilization review and discharge planning solutions, according to a new report by EHRIntelligence.
The change is intended to improve the care coordination services that the companies offer to healthcare organizations and payers. To date, Allscripts’ platforms have been employed by approximately 1,000 hospitals and 20,000 providers in the United States.
“Outcomes-based reimbursement models created a heightened need for providers and health organizations to transition patients out of the hospital efficiently,” said CarePort Health’s CEO, Dr. Lissy Hu.
The ongoing shift toward value-based care also requires providers to better “track patients’ health as they transition across multiple care settings,” she said.
