Where do homeless patients go after they are discharged from acute care? Typically, back to the streets, without proper nursing support or outpatient monitoring. Often, such situations result in additional emergency care, rebound admissions and avoidable, costly inpatient care.
Commissioners in Los Angeles County are expected this week to weigh that cost against a $10.1 million population health initiative that seeks to reduce the impact of negative social determinants of health (SDOH) like homelessness and indigence.
The plan calls for the county to fund renovation of a soon-to-be decommissioned building on the Rancho Los Amigos National Rehabilitation Center’s campus into a new center that would provide basic recuperative care at a fraction of the cost of full inpatient care.
