A recent survey conducted by Humana and the Healthcare Financial Management Association (HFMA) revealed that 70% of those executives surveyed believe that quality data sharing is directly tied to value-based care success. A majority of executives also acknowledged the need to improve post-discharge follow-up, chronic care management and data access.
